Position Summary:
The RN provides direct nursing services to Axis Health System patients, and clinical and administrative support to medical providers. Nursing duties include preparing for patient admissions and visits, obtaining and documenting vital signs, assisting medical providers with procedures, administering medication and immunizations, providing health education, chronic care management and preventative care. Assigned to a primary location, but may be required to assist at other clinics.
Qualifications: Our ideal candidate for our Nurse, RN position will have graduated from an accredited school of nursing and be licensed as a Registered Nurse in the State of Colorado. Preferred Education: Bachelor’s Degree. They will have2 Years of Experience in a health care setting Preferred Experience: 2 years Nursing experience.
Salary:
Starting pay is $37.00 (Base Pay $36.00 + Locale Diff $1.00) – $40.66 annually and considers preferred education, experience and licensure.
*This position includes a $1.00/hr. locale differential for our Montrose and Delta worksites.
Nursing Core Job Duties:

Provides nursing direct care (acute and chronic) to a diverse population of patients. This includes but is not limited to nursing assessments, care coordination, direct nursing intervention, testing, procedures, phlebotomy and patient education.

Completes initial screening and triage for patient concerns using approved triage protocols.

Utilizes the seven rights of medication administration when administering medications and immunizations.

Maintains knowledge of the uses and effects of medications and immunizations, including long acting injectable medication.

Ensures all orders submitted by medical provider are completed within expected timeframes.

Regularly assists providers with chronic care management and care plan updates for patients who have complex needs.

Responsible for ensuring infection control procedures and best practices are followed, which may including training and supporting other clinic/unit staff.

Completes all clinical documentation within expected timeframes.

Supports clinic/unit workflow by monitoring patient census and schedules, preparing for visits, assisting with urgent orders and referrals, and delegating tasks to the Medical Assistants or Behavioral Health Techs as indicated.
