Schedule Options
We offer a variety of schedule options to accommodate different preferences and needs. These include:
• 8-hour Evening shifts
• 8-hour Night shifts (or Rotate 8 E/N)
• 12-hour Day shifts
• 12D/12N Rotation shifts
• 12-hour Night shifts
• Baylor weekend positions for enhanced pay
A collaboration with our Hospitalist, Pharmacy, Education, Care Coordination department, and clinical staff work closely to provide individualized evidenced-based interventions for patients with a variety of medical, cardiac, and respiratory illnesses including sepsis, pneumonia, CHF, COPD, and prolonged ventilator and tracheostomy care. Soon we will roll out our new and advanced Virtual Connected Care™ Together Team nursing model that integrates experienced telemedicine RN ‘s into care assignments, thus helping with the workload. This innovative care model allows for superior patient monitoring, provides expert nursing consultation/support, and enhances the patient and staff experience.

Responsibilities:
- Responsible for collaboration, communication & facilitation of optimal provision of care with assistance of computerized &/or digital platform that supports the daily management of care.
- Responsible for the assessment, planning, implementation, coordination, monitoring & evaluation of the patient's plan of care from admission to discharge.
- Utilize clinical knowledge, critical thinking skills & the principles of case management & adult education to coordinate, implement & support the plan of care.
- Ensure a seamless, effective, efficient transition of care across the continuum.
- Monitor quality metrics specific to the department & ensure complete & accurate documentation in the patient record.
- Act as 1) mentor; 2) clinician; 3) educator; 4) consultant/collaborator; and 5) navigator.
Education, Training, Experience, Certification and Licensure:
- Graduation from an accredited school of nursing and current licensure in the State of New York with at least 1 year of experience.
- Baccalaureate Degree in the Science of Nursing (BSN) degree from an accredited school of nursing preferred, according to NYS requirements.
