Job Summary:
This position is a registered nurse who supports hospitalized patients through care coordination, clinical review, and discharge planning activities throughout their acute care stay using virtual technology. Operating from Virtua’s centralized command center, this role collaborates with patients, families, and interdisciplinary teams to promote safe, efficient, and well-coordinated transitions of care.
Focuses on reviewing clinical information, identifying potential barriers to discharge, supporting medication accuracy and reinforcing education, while working in partnership with bedside clinical teams. When patients are enrolled, the nurse coordinates education, logistics, and transition support and may provide onsite assistance to ensure a smooth and safe transition from the hospital to home. This role delivers care virtually while maintaining the highest standards of safety, quality, and patient experience across multiple acute care locations simultaneously.
Position Responsibilities:
Conduct admission-level reviews of newly hospitalized patients to support medication reconciliation processes, ensuring accuracy of documented information. Identify and escalate potential safety concerns, documentation gaps, or coordination needs according to organizational standards.
Perform ongoing clinical and discharge readiness reviews to identify barriers to transition, including social, logistical, or system-related factors. Collaborate with interdisciplinary teams to support discharge planning and assess eligibility for programs such as Hospital at Home. Facilitate communication and coordination but does not replace bedside clinical decision-making.
Support discharge preparation activities by reviewing documentation for completeness, reinforcing patient and caregiver education related to medications, equipment, and follow-up care, an ensuring alignment across care teams for safe transitions.
Maintain compliance with HIPAA, confidentiality, and regulatory standards.
Participate as an active member of the care coordination team and maintain competency in required virtual care technologies.
