Patient Nurse Navigator

When a patient requires care from different facilities and/or practitioners they need a Patient Nurse Navigator to identify needs and remove any barriers to timely, appropriate care. Our Nurse Navigators guide the patient from diagnosis through treatment and act as the central point of contact for the patient and the entire medical team. It’s a job that requires a strong communicator with excellent organization skills.

WHAT YOU CAN EXPECT
- A didactic learning experience in a fast-paced environment.
- Identifying patients who need specialty care through clinical analysis for Franciscan primary care physician offices and hospital services
- Gathers and integrates patient, nursing, or information systems data from different sources/systems; analyzes and interprets to improve Clinical services.
- Provides clinical resources guidance on where to seek a second opinion, treatment or testing that may not be available locally, and how to access reliable sources of information.
- Assess patients for psychosocial distress and referral to a medical social worker or counselor if support that is more intensive is required.
- Patient & physician Liaison Answers questions as appropriate and serves as liaison to the physician for additional questions and concerns.
- Addresses tangible barriers to care such as finances, housing, transportation, childcare, prescription costs, and much more. Refers patients to the appropriate contact, such as a social worker, for support needed.
- Ensures any health information provided is accessible, such as securing documents printed in the patient’s own language or arranging for certified medical interpretation, either in-person or by phone/videoconference.
- A member of an interdisciplinary team to assess, secure, and coordinate appropriate services through the continuum of care.
- Empowers patients, families, and caregivers to assume as much responsibility for their care as possible, within the constraints of age, illness, and other factors.
- Educates patients about their disease, prescribed treatment, and side effects of treatment and how to manage them.
QUALIFICATIONS
- Preferred Bachelor's Degree Nursing
- 1-year related experience, 3+ years Preferred
- Cardiology or Cardiovascular experience Preferred
- Registered Nurse (RN)
- Basic Life Support Program (BLS) - American Heart Association
- Experience in Software clinical data analysis
