Pediatric Home Health LPN/RN

Family First Homecare (FFHC) is seeking a compassionate and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide high-quality, in-home nursing care to pediatric patients. In this vital role, you will:

Key Responsibilities

  • Provide nursing care within the scope of your licensure and clinical competencies
  • Maintain up-to-date, accurate clinical records for each patient, meeting all documentation deadlines
  • Continuously evaluate the effectiveness of nursing care for patients and families
  • Consult with physicians regarding changes in patient care plans and document all updates
  • Communicate changes in patient condition to the nurse supervisor
  • Support FFHC’s mission by mentoring and/or coaching new employees during patient-specific orientations
  • Educate patients and caregivers on disease processes, treatments, and home care needs
  • Participate in FFHC’s Quality Assurance and Performance Improvement (QAPI) program as requested
  • Actively seek opportunities to improve clinical skills and knowledge
  • Attend and/or complete all mandatory trainings and in-services
  • Submit required personnel file documentation in a timely manner
  • Adhere to FFHC compliance and ethics expectations
  • Maintain patient and family confidentiality at all times
  • Demonstrate professional, supportive, and responsive communication skills
  • Perform other related duties as assigned

Qualifications

Required

  • LPN/LVN Nursing Degree
  • Unencumbered LPN/LVN license in the state of employment, or multi-state license through the Enhanced Nurse Licensure Compact (where applicable)
  • Minimum 1 year of nursing experience
  • Valid driver’s license with proof of insurance (in applicable states)
  • Ability to pass all required background screenings
  • Working knowledge of state Nurse Practice Acts
  • Valid and current CPR certification with in-person, instructor-led skills check
  • Clear Hepatitis B screening (in applicable states)
  • Clear TB screening (in applicable states)
  • Ability to read and communicate in English, both verbally and
