Case Manager
The Case Manager provides supportive services to chronically homeless individuals with disabilities by collaborating with case managers on benefits, budgeting, and housing stability, maintaining compliance documentation, coordinating community outreach and volunteer engagement, and representing the agency in local initiatives.
Job Responsibilities

Provides supportive services for chronically homeless individuals with a documented disability by working collaboratively with the client's case manager to obtain income and benefit verification, create and maintain client budget, and document client monthly rent and utility payments.

Assist clients in completing state/federal entitlement benefit applications and advocates for them, as needed.

Maintains up-to-date accurate client files and databases in compliance with all Agency/programmatic policies and procedures, professional standards and external requirements of all relevant regulatory, licensing or accrediting bodies.

Participates in community outreach efforts, by establishing partnerships with community resources and volunteer opportunities. Works with partners to match clients with prospective volunteer opportunities. Maintains a positive relationship with community partners.

Represents the Agency at Lake County functions committee meetings as assigned.

Coordinates with Case Managers to plan three client engagements per year.
