As a Clinical Coordinator at Transitions Care you are accountable for the patient census regarding transitional care management, care coordination, and continuity of care. The Clinical Coordinator will have a vital part in coordinating care including scheduling, supply ordering, DME and any clinical concerns.
Major Responsibilities
- Accountable for facilitating an interdisciplinary approach to patient care
- Provides support to case managers of their Pod
- Assumes responsibility for meeting regulatory and other mandatory requirements
- Promote exemplary customer service to providers, facility personnel, patient, and patient family at all times
- Assist with refill requests and prior authorizations.
- Coordinate with orders as it relates to home health- admission, discharge, 485, transfer, documentation requests
- Reviews, interprets, transcribes, and carries out physician orders
- Manage lab orders (pt/inr) and coordinate with provider on treatment plan
- Assist in answering clinical calls and questions from patients, home health agencies providers, etc
- Documents all communication appropriately in the EMR (telephone encounter, CCM template, etc.)
- Contacts patient post hospitalization or skilled nursing facility discharge to initiate Transitional Care Management and follows up with scheduling to ensure patient is on schedule within 7 days post discharge

Requirements
Education and Experience
- One (1) + year of home care or hospice experience
- Minimum of LPN Required
- Strong knowledge of home health regulations, Medicare requirements and care coordination skills required.
- Microsoft office and excel proficiency required.
- Excellent customer service, organization, and time management skills required.
Specific Proficiencies
- Understanding of palliative philosophy, principles of death/dying
- Proven ability to work in an interdisciplinary setting
- Self-directed with the ability to work under minimal supervision
