Job Summary:
Join our compassionate and collaborative team as the Assistant Director of Healthcare Home, where you will play a vital role in overseeing and supervising the daily operations of the HCH Team. This position is essential in coordinating and tracking HCH clients, ensuring they receive the best care possible. You will have the opportunity to lead health education groups and activities, making a significant impact on the lives of our clients and the community. We are looking for a dedicated individual with strong leadership skills and a passion for helping others.

In this role, you will supervise Nurse Care Managers, coordinate health education initiatives, and serve as the Cyber Access Practice Administrator. Your leadership will help shape the future of healthcare in our community.

Key Responsibilities:

  • Oversee and supervise the daily operation of the HCH Team.

  • Coordinate and track HCH enrollees, including new enrollees, discharges, and transfers.

  • Supervise Nurse Care Managers and assign NCM caseloads.

  • Facilitate health education groups and activities for clients, staff, and the community.

  • Review, complete, and submit the monthly Attestation report in CyberAccess in a timely manner.

  • Serve as a Cyber Access Practice Administrator, maintaining user accounts and monitoring agency usage and prescriber reports.

  • Manage HIT tools and initiatives, including CyberAccess usage, CMT reports, and hospital admission/discharges.

  • Facilitate staff training on Healthcare Home.

  • Participate in monthly TA calls for HCH Directors and attend state-required meetings and trainings.

  • Oversee Quality Assurance and Quality Improvement for Healthcare Home.

Education, Experience, and/or Credential Qualifications:

  • BSN or MSN

  • Registered Nurse

  • Supervisory Experience

Additional Qualifications:

  • Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check.

  • Completion of New Hire Orientation at the beginning of employment.

  • All training requirements including Relias at the beginning of employment and annually thereafter.

  • Current driver’s license, acceptable driving record, and current auto insurance.

Physical Requirements:

  • ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up to 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.

  • Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
