About the role

Coordinates Core measure abstraction, including collection, analysis, and reporting of data and improvement of clinical quality indicators. Reports to the Manager of Quality and Performance Improvement.

Education :

  1. Bachelor's degree or Associates degree in Nursing

Work Experience :

  1. Must have a minimum of three years of experience in working in a Quality/Performance Improvement program or satisfactory nursing or paramedical background. Good oral and written communication skills required.

    1. Job Function :
      1. Coordinates functions of the Core Measure abstraction including data collection, data submission, daily quality report issues, outcomes report review and analysis. Works with physician and management leadership to identify and coordinate Performance improvement efforts based on outcomes reporting. Compares CMS data to data collected for core measures to ensure alignment of results. Based on a thorough understanding of the core measure components, responds to all requests for information to include: Clinical interpretation of data element definitions; Data submission process; Data quality; Comparative Institutional Report interpretation; and Use of outcome reports for quality measurement and improvement Develops training and provides education related to improving performance on clinical quality indicators and for delivering training and education to staff, departments, physicians. Ensures abstracted clinical data is valid, reliable, and can be accessed rapidly. Maintains information system security and patient confidentiality. Meets deadlines for abstraction and submission of data as established by internal expectations and external regulations.
      2. Prolonged periods of standing, walking, or sitting while on duty. Lifting, pushing, and pulling up to 10-15 pounds with or without assistance. Visual and auditory acuity and manual dexterity essential to performing designated duties required. Ability to perform effectively in a stressful and fast paced environment. Ability to pass all required health and other screening tests. Physical conditions are clean, neat and well lit. Maybe subjected to unpleasant sights and odors, stressful situations and hazardous or infectious agents where judgment as to precautions need to be taken is essential. Climate control an ambient temperature variances may be experienced. Exposure to limited amounts of hazardous chemicals or substances and infectious disease processes are a possibility. Direct contact with blood or other body fluids to which universal precautions apply. Personal protective equipment is made available and must be worn. Computer skills preferred and willing to learn, Availability for out of the office meetings and conferences. Interacts one non one or in group settings with all staff levels, including physicians. Hours of duty may be irregular or unexpectedly extended due to emergency circumstances.
