RATE OF PAY/SALARY: $30.21 per hour

WORK LOCATION(S): Newburgh, NY

STATUS: Full Time

  •  Provides comprehensive case management services to HIV positive clients. 
  •  Provides a client-centered approach that links clients with health care, psychological and social services in a manner that ensures timely, coordinated access to medically appropriate levels of care, support services and continuity of care.
  •  Provides follow-up and outreach for those clients not returning for care.
  •  Provides documentation to reflect client interaction, outreach attempts, biannual psychosocial assessments and service plans. 
  •  Engages client in risk reduction and behavior change strategies. 
  •  Provides partners of HIV + clients with HIV/AIDS education and risk reduction strategies. 
  •  Provides case management input and psychosocial information concerning patients during medical conferences.
  •  Conducts hospital and home visits to patients. 
  •  Establishes peer support groups and focus groups for our clients.
  •  Submits timely documentation of case management services. 
  •  Refers patients to programs, support groups and medical treatment, as needed.
  •  Provides Pre and Post testing and counseling for GHVFHC patients.
  •  Develops linkages and networks with other groups and agencies offering services to HIV clients to help promote cooperation and reduce duplication of services among agencies.
  •  Attends conferences and various training sessions, as needed.
  •  Performs updates and maintenance of patient charts.
  •  Assists in CQI projects for program development.
  •  Develops relationships and represent GHVFHC in community organizations. 
  •  Performs other related duties as discussed and assigned

Requirements

  • Must be Bilingual (English/Spanish)
  • Bachelor of Science/Art in Social Work, Psychology, Human Services or related 2-3 years field experience.
  • Highly organized with excellent oral and written communication skills.
  • Ability to maintain a nonjudgmental disposition and communicate with a diverse population.
