This position is an individual charged with their development of clinical expertise. The Advance Practice Professional (APP) Fellow completes comprehensive patient health evaluations (including biological, social, and psychological assessments) as assigned by appropriate clinical/faculty team. The APP Fellow assesses and coordinates the educational and psychosocial needs of the patient and caretakers. The APP Fellow develops and reviews the plan of care with the other members of the clinical team, participates in directing that care, and coordinates peri-operative care, discharge planning (as applicable) and appropriate post hospital and ambulatory follow up. The APP Fellow will participate in educational curriculum as outlined in the program.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelor ’s Degree as Physician Assistant or Master ’s Degree as Physician Assistant required. 

2. Graduate of an ARC-PA approved Physician Assistant (PA) training program required.

3. Certification by the National Commission for Certification of Physician Assistants required.

4. Must be licensed or eligible for licensure by the WV Board of Medicine or WV Board of Osteopathic Medicine.

5. American Heart Association BLS Certification required with additional Advanced Life Support training at the discretion of the department.

EXPERIENCE:

1. Eligible new graduates will be considered.

1. Master’s level degree as Physician Assistant preferred

EXPERIENCE:

1. Clinical experience in fellowship specialty preferred. 

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position.  They are not intended to be constructed as an all-inclusive list of all responsibilities and duties.  Other duties may be assigned.

1. Manages comprehensive and individualized care for patients and their families in the inpatient phase of hospitalization (as applicable), immediate post discharge, and in the ambulatory setting in conjunction with Fellowship leadership and/or the care team.

2. Completes daily rounds; consults and/or pre-procedure evaluations as assigned.

3. Completes integrated assessments as assigned.

4. Participates in peri-operative management of patient as assigned.

5. Orders additional diagnostics or consults as deemed appropriate with the collaboration of  providers or by following established Fellowship guidelines.

6. Performs and communicates results of diagnostics following established Fellowship guidelines and scope of practice.

7. Consults and communicates with clinic and hospital staff regarding abnormal findings in the evaluations of patients.

8. Performs succinct comprehensive clinical evaluations and documents findings in medical record as assigned.

9. Facilitates patient discharge/follow up by coordinating appropriate services and diagnostics prior to discharge necessary for outpatient follow-up appointments in the clinic (as applicable) as assigned. 

10. Performs necessary procedures following established Fellowship guidelines for scope of practice (SOP). 

11. Develops a comprehensive individualized plan of care for patients, which includes teaching, discharge planning/follow-up and necessary referral to other members of the health care team as assigned.

12. Demonstrates expertise in caring for patients across the age continuum of their educational experience and certification.

13. Participates in patient rounds and consultations (as assigned). 

14. Identifies unstable or complex patient care situations and communicates to Fellowship leadership and/or clinical providers to initiate appropriate intervention in the clinic and/or hospital setting.

15. Facilitates patient discharge and/or postoperative care by coordinating appropriate services and diagnostics prior to scheduled outpatient follow up appointments (as assigned). 

16. Assists in the collection of outcome measures (as assigned).

17. Recognizes emergency situations and initiates effective emergency care in collaboration with clinical team.

18. Evaluates the effectiveness of the patient/family teaching and collaborates with care team in the completion of education plan.
