Position Description:
The Health Planning Specialist III supports the development and coordination of community-based public health programs. This role leads specific community engagement initiatives, supports CHA/CHIP facilitation, builds strategic partnerships, and assists with program evaluation and grant support.
Position Responsibilities/Essential Functions:
The functions within this job family will vary by level, but may include the following:
Lead coalition-building efforts to support public health programs and wellness initiatives.
Independently coordinate portions of the Community Engagement and Planning team's activities.
Lead data collection, analysis, and reporting for CHA/CHIP processes.
Conducts program and community need assessments to inform strategic priorities.
Assist with grant implementation, reporting, budget tracking, and purchasing.
Develop and implement community outreach and communication plans in collaboration with team leadership.
Establish and maintain strong external partnerships to enhance public health initiatives.
Provide consultation and support to internal staff, community partners, and coalition members.
Prepare and present programmatic reports with findings and recommendations for improvements.
Support community engagement and health planning team in all community engagement activities.
