POSITION: SITE SUPERVISOR
CLASSIFICATION CODE: EXEMPT FULL TIME
DEPARTMENT: LIVING WITH PURPOSE
REPORTS TO: LIVING WITH PURPOSE PROGRAM MANAGER
STATEMENT OF JOB: The Site Supervisor works closely with the other Site Coordinators, Case Managers, Clinicians and Program Manager in the Living With Purpose program to provide responsive, high quality services and programs in the White Mountain Apache Tribe under the authority of Apache Behavioral Health Services. The Site Supervisor for the Living With Purpose program provides employment support services oversight, guidance, transportation and other related services to support client independence and retention. The Site Supervisor supervises Living With Purpose work projects, transports clients and is responsible for the overall management of Living With Purpose staff engagement ensuring the highest quality of completed work projects.
DUTIES AND RESPONSIBILITIES:
- Provide employment support services to increase occupation skills development and workforce retention.
- Transport clients to appropriate services and work project sites as needed.
- Assist in the implementation of scheduled Living With Purpose employment support services evaluations. Conducts ongoing assessment to identify skill or knowledge gaps and compliance requirements needing to be addressed.
- Provide individualized occupational support guidance on such topics as time management, teamwork, goal setting, work ethic and professional behavior.
- Coordinate with Living With Purpose Site Supervisors, Case Managers, Therapists and Program Manager to identify and refer high risk individuals to appropriate support services and community resources.
- Work with Living With Purpose Site Supervisors, Case Managers, Therapists and Program Manager to assist with the coordination, scheduling, outreach, and promotion of the Living With Purpose work project offerings.
- Oversee work project sites to monitor attendance, ensure a physically and emotionally safe work environment and timely project completion.
- Maintain inventory of supplies and equipment. Submit requests for order or purchase of necessary supplies or equipment to Program Manager.
- Maintain accurate records, develop reports and documents as requested.
- Abide by all applicable policies, procedures, and program standards of ABHS, White Mountain Apache Tribe, Indian Health Services, Arizona Health Care Cost Containment System (AHCCCS) and other pertinent regulatory authorities.
- Maintain ethical policies and guidelines as outlined in accepted State ethics laws, rules, policies and principles and as outlined in discipline-specific code of ethics.
- Provide a positive role model to staff through leadership, self-discipline, and genuine care for the treatment and recovery of clients. All staff members must be treated with dignity and respect.
- Participation in all required ABHS training.
- Reports/attends work in timely manner. Must be reliable and communicate directly with supervisor in all matters of attendance.
