Job Summary:

St. Elizabeth Healthcare is hiring a Certified Nurse Assistant for our Home Hospice.

Shift/Hours: Full Time, 40 hours/week, 1st shift, 8A-430P, Monday-Friday with one Saturday shift every 6 weeks.

Provides direct and indirect patient care under the supervision and direction of the RN case manager.  Assists patients with activities of daily living (ADL), provides for personal care, comfort and assists in the maintenance of a safe and clean environment for an assigned group of patients.

Demonstrate respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.

• Patient Care Ability to provide all ADL’s for Hospice patients
• Communication Reports pertinent information such as changes in client condition or behavior to the case manager or Assistant Nurse Manager.  Understands and has ability to follow the C.N.A. plan of care as directed by the case manager.
• Interdisciplinary Team Participation Attends IDT meetings when required and communicates significant information as it relates to personal care.
• Documentation Documents completely and accurately on itinerary and visit records. Documentation turned into the office within 24 hours of visit.
• Personal Growth & Development Attends and seeks out in-services to maintain certification.  Has knowledge of regulatory requirements as related to position.
• Performs other duties as assigned.

Education, Credentials, Licenses:

• High school or GED graduate.
• Certified in the state where work is being performed as Certified Nurse Assistant.
• Not listed on the applicable State Abuse Registry.
• Ability to read and communicate effectively in English.

Specialized Knowledge: 

• Demonstrates basic mathematical skills.
• Basic life support.
