Position Summary:
The Admissions Nurse (RN) is responsible for conducting comprehensive in-home assessments and initiating the plan of care for patients admitted to Health Link’s home health program. This role ensures a seamless and safe transition from hospitals, skilled nursing facilities, or physician offices to home health services. The Admissions Nurse works collaboratively with interdisciplinary teams to ensure patient-centered, high-quality, and timely care.
Key Responsibilities:

Conduct initial patient assessments in the home setting, including physical, psychosocial, environmental, and health status evaluations.

Initiate the individualized plan of care based on patient needs, physician orders, and regulatory guidelines.

Educate patients and families on home health services, medications, disease process, and self-care techniques.

Collaborate with physicians, case managers, and other team members to coordinate timely and appropriate admissions.

Ensure accurate and timely documentation in the electronic health record (EHR) system, compliant with Medicare/Medicaid and agency standards.

Identify and address barriers to care; facilitate referrals to appropriate services or resources as needed.

Ensure compliance with state, federal, and accrediting body regulations during the admission process.

Participate in on-call rotation and provide support to field staff when needed.
Qualifications:

Education: Graduate of an accredited school of nursing; BSN preferred.

Licensure: Current Registered Nurse (RN) license in CA; must be in good standing.

Experience: Minimum of 2 years of clinical nursing experience; at least 1 year in home health or admissions preferred.

Certifications: CPR certification required; working knowledge OASIS certification preferred.

Skills:

Strong assessment and clinical decision-making skills.

Knowledge of home health regulations (CMS, Medicare).

Excellent communication, documentation, and interpersonal skills.

Proficient in the use of EHR systems and mobile technology.

Ability to work independently and travel within service areas.

Physical Requirements:

Must be able to drive and travel to patient homes.

Able to lift, position, or transfer patients as needed.

Requires long periods of standing, walking, and performing clinical tasks.
