POSITION SUMMARY:
 
The Community Health Worker (CHW) plays a vital role in supporting California’s Medi‑Cal transformation under CalAIM (California Advancing and Innovating Medi‑Cal) by providing relationship‑based, whole‑person support grounded in lived experience. As a trusted member of the community, the CHW builds meaningful relationships, delivers culturally responsive health education, supports care coordination, and helps individuals and families navigate health and social service systems, including CalAIM Enhanced Care Management and Community Supports. The CHW works collaboratively with the World Relief CalAIM team, healthcare providers, and community partners to reduce barriers to care, address social drivers of health, and empower clients and improve health outcomes for the communities they reflect and serve. 
 

ROLE & RESPONSIBILITIES:

    Outreach & Engagement  
  • Actively locate, contact, and engage clients who have been identified as eligible for CHW services.    
  • Use culturally and linguistically appropriate outreach strategies to build trust and establish rapport.  
  • Conduct in-person meetings with clients in the community or at provider locations to assess their needs and preferences. 
  • Document all outreach and engagement efforts thoroughly and in a timely manner.  
  • Health Promotion & Education  
  • Use motivational interviewing, trauma‑informed, and harm‑reduction approaches to support clients in accessing healthcare, understanding the healthcare system, engaging in their own care, and connecting to community resources that promote health and well‑being.  
  • Lead or help facilitate community workshops and group activities on topics such as healthy relationships and communication, nutrition, stress management and mental health awareness, substance use and tobacco cessation, violence prevention and coping skills, child and adolescent immunizations, housing resources, and navigating the healthcare system and public benefits.  
  • Develop and distribute health promotion materials.  
  • Act as a liaison between healthcare, behavioral health, housing, and community‑based providers to support coordinated and integrated services.  
  • Assist clients with scheduling appointments, completing applications, and arranging transportation as needed.  
  • Support clients in building daily living skills and long-term self-sufficiency.    
  • Coordination & Referrals  
  • Provide trauma‑informed care coordination and referrals, recognizing the impact of trauma and prioritizing safety, choice, trust, and empowerment in all client interactions.  
  • Connect clients to essential needs such as housing resources, food pantries, transportation, and public benefits.  
  • Facilitate access to CalAIM services for eligible clients, including Enhanced Care Management and Community Supports, and collaborate with the CalAIM team to support coordination of services once clients are enrolled.
