Requirements:
• Current California State NP or PA license
• Current Nurse Practitioner Furnishing Number (for Nurse Practitioners)
• Nurse Practitioner furnishing number of NPs (for Nurse Practitioners)
• CAPA Controlled Substance Certification (for Physician Assistants)
• Active DEA registration
Preferred:
• 6 months or more of related clinical experience in primary care
