The Opportunity

As One Medical for Seniors High Risk Programs continues to expand, we’re looking for energetic and passionate leaders to manage the people, processes, and technology that make One Medical unique. As a Senior Manager, High Risk Care Field Operations, reporting to the Director of High Risk Care Programs, you’ll be responsible for meeting One Medical patient promises of outstanding quality of care, access, and truly patient-centered experiences at an affordable cost. You’ll execute our mission of delivering high-quality care and service using rigorous performance management and improvement systems found within The One Medical Performance System (TOPS).  Specifically,  you’ll lead the operations of a primary care house calls program and related field based services operating currently in five states, across four time zones, serving the highest risk patients of our Senior Health population.

What you'll likely work on:

  • Lead the management of the day to day operations of a program which includes virtual care teams supporting in-market/ in-home primary care  house calls providers 
  • Lead the design and implementation of standard work, partners with other High Risk Care and company Leaders to help determine operational strategies by evaluating team results and objectives, and maintains and improves our operations through continuous improvement 
  • Lead the continuous risk stratification and patient identification processes and the resulting patient enrollment processes that assure program reach and impact for the highest risk of the patient population 
  • Ensure all patients are receiving the appropriate outreach, Health Coach support, visit scheduling  and care coordination during their enrollment and participation within High Risk Care programs
  • Build an engaged team leveraging CICARE (a framework containing the key elements of a great interaction and effective communication that we use with patients and each other), Active Daily Management, and Lean principles to meet patient care and team experience goals
  • Actively communicate and partner with market and in-office care team leaders to assure clear visibility of the High Risk Field Operations team performance to the service agreement and effective handoffs for patient care
  • Travel to in-market locations of the High Risk Care field operations on a bi-monthly to quarterly basis.

What you’ll need:

  • 3+ years leadership experience within programs serving high risk patient populations including care through a home based care model
  • 5+  years experience in operations and process management
  • 3+ years experience in direct people management; virtual team management experience preferred
  • Deep knowledge of value based care operational context, high risk care services and home based primary care/ house calls operational models
  • Proven ability to drive results and support change management and continuous improvement 
  • Proven ability to foster strong, collaborative peer relationships or team team-dynamics that ensure a supportive and engaged team culture
  • Experience developing talent through mentorship and coaching, consistent feedback, goal setting and accountability   
  • Proficient
