Job Description:

Works with patient populations identified as “at-risk” in collaboration with their interdisciplinary care team. Identifies and assists in closure of gaps in care related to preventive care, chronic disease care and regulatory services. Assists in delivery of high quality, efficient, and cost-effective healthcare services. Under general administrative direction, gives patient education related to their condition and planned treatment, promote patient self-management, suggests care plans with providers, manages transitions of care, etc.

• Current RN license in the state of North Dakota, Minnesota, and/or Wisconsin upon hire. Ability to obtain RN license in all 3 states within 3 months of hire
