Licensed Practical Nurse (LPN)

The LPN provides professional nursing care and coordination of nursing care by working cooperatively with patients, family members, and all patient care team members. These services are for adult patients of all ages in our Manhattan area clinics.

This position travels between several Manhattan area clinics. LPN positions are available in all New York areas including Bronx, Queens, and Brooklyn.

Responsibilities

• Documents patient’s physical assessment, vital signs, patient’s treatment reviews and observations in patient medical records; brings issues to the attention of the physician and primary nurse.
• Communicates emergencies, patient life-threatening complications, vascular access issues, and related incidents to RN, nurse manager, and the patient’s physician.
• Documents findings of patient assessments and interventions and advises providers of any significant change in the patient’s condition and other pertinent information.
• Assess, prepares, and cannulates a patient’s vascular access.
• Provides patient education/teaching applicable to the patient’s medical condition and charts training and patient description or demonstration of training received.
• Attends and leads facility orientation, meetings, and mandatory in-services as required.
• Inform Administrator and Medical Director of all incidents, conditions, problems, counseling regarding teammates, patient care, or facilities.
• Assists the Administrator in quality measures, i.e., audits, infection control compliance, accreditation preparation, etc
• Verifies accurate charges and documentation are submitted in a timely manner.
• Additional duties as assigned.

Requirements

• Current LPN license in the state of New York required.
• Current BLS certification through American Heart Association or The American Red Cross or equivalent – required.
• IV push medication certification – required.
• Must be able to work in several clinics within a designated area.
