As our Social Work Supervisor in the PACE program, you’ll get the best of both worlds — guiding and uplifting an amazing team of social workers while continuing to connect directly with participants through your own manageable caseload.
In this role, you’ll be a mentor, coach, and cheerleader, providing thoughtful feedback and support to help your team grow and deliver outstanding care. You’ll champion high-quality service that aligns with PACE standards and truly makes a difference in participants’ lives.

What Does Success Look Like As Our Social Work Supervisor?

  • Inspire and empower your team by guiding, mentoring, and supporting a dedicated group of Social Workers, while ensuring alignment with PACE standards and social work best practices.
  • Champion growth and development through regular feedback, thoughtful coaching, and one-on-one meetings that help your team thrive and make meaningful impacts every day.
  • Stay hands-on and connected by managing your own caseload, delivering biopsychosocial assessments, personalized care planning, and comprehensive case management.
  • Be a community connector by linking participants to vital resources such as housing, transportation, and financial assistance to support their overall well-being.
  • Strengthen support networks by collaborating closely with family members and caregivers, fostering continuity of care and deeper participant relationships.
  • Lead with accountability by ensuring all documentation is timely, accurate, and compliant with CMS, DHCS, and PACE regulatory standards.
  • Drive positive change by contributing to quality improvement initiatives and supporting audits or surveys to continually enhance participant outcomes and experiences.
  • Collaborate across disciplines as an active participant in IDT meetings, helping to shape integrated, participant-centered care plans focused on health, safety, and well-being.
  • Create a culture of warmth and respect by fostering open communication, shared accountability, and a supportive, team-oriented atmosphere.
  • Pitch in where it counts by stepping up to support other duties and responsibilities as needed — we’re all in this together!

What Does An Ideal Candidate Look Like?

  • Bringing a strong educational foundation, with a Master’s degree in social work (MSW) from a Council on Social Work Education-accredited program — setting the stage for exceptional participant care.
  • Holding a valuable professional license, with an active California LCSW preferred to help guide and shape high-quality social work practice.
  • Drawing on rich experience, with at least 5 years in social work and at least 1 year in a supervisory or leadership role to inspire and support your team with confidence.
  • Showing a deep passion for older adults, with experience (ideally at least 1 year) working with seniors, medically complex populations, or individuals with chronic conditions.
  • Leveraging expert knowledge, including a strong understanding of community resources, Medicaid/Medicare, and public benefit programs to help participants thrive.
  • Leading with exceptional interpersonal skills, using strong communication and conflict resolution abilities to create a trusting, collaborative environment.
  • Demonstrating a mission-driven spirit, showing an unwavering commitment to serving high-risk seniors and frail older adults with compassion and respect.
  • Staying connected and mobile, with a valid California driver’s license so you can actively engage with participants and community partners.
