WeCare at Foltsbrook is actively seeking motivated Licensed Practical Nurses for our community in Herkimer, NY. Our Licensed Practical Nurses assist in ensuring the general health and well-being of our residents by providing nursing care. The Licensed Practical Nurse (LPN) provides nursing care under the supervision of a registered nurse (RN) or physician to residents. The LPN assists in implementing the nursing care plan, administering medications, and providing basic medical care to residents.

Key Responsibilities

·      Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.·      Provide regular patient status updates.·      Assist in implementing patient’s treatment plan under direction of the Registered Nurse.·      Complete required documentation in an accurate and timely manner.·      Supervise, administer and or assist with medications.·      Provide direct care of residents as necessary.·      Follow the plan of care for each resident.·      Ensure the residents care standards are maintained.·      Report to the Director of Nursing.

Skills, Knowledge and Expertise

·      Valid NYSED LPN Certification·      Basic Life Support – American Heart Association (BLS)
