Home Health Physical Therapist or Physical Therapist Assistant (PRN)

Responsibilities

• Provide one-on-one, in-home visits to patients requiring physical therapy
• Evaluate and treat patients with a variety of diagnoses, including neurological, orthopedic, and cardiopulmonary conditions
• Assess patient needs and develop an individualized plan of care
• Educate patients and their families about the goals of treatment and home exercise programs
• Document patient progress and communicate regularly with interdisciplinary healthcare team
• Adhere to all applicable laws, regulations, and ethical standards of practice
• Maintain accurate and up-to-date patient records

Requirements

• Physical Therapist or Physical Therapist Assistant degree from an accredited institution
• Current state license as a Physical Therapist or Physical Therapist Assistant in Michigan
• Minimum of 1 year of clinical experience, ideally in home health care, is preferred, but not required
• Experience with diverse patient populations
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Valid driver's license and reliable transportation, as frequent travel is required
