Serves as the dedicated liaison for patient access improvements and workflow education as it relates to ambulatory access and patient partnership at the local practice level. Partners closely with department leadership to promote and accelerate enterprise access goals. Works closely with EMR Operations and administrators to facilitate implementation of access workflows and education. Recognizes and alerts opportunities for improvement and assists in determining countermeasures.Job Description
- Coordinates patient access by assisting with appointment scheduling across entire enterprise, escalating issues, and advocating for patients.
- Serves as team member to support strategic plans, goals, and strategies with particular emphasis on patient access and patient partnership.
- Analyzes access data and provides updates/reports as well as trends noted on tracking of referral and access data to practice leadership.
- Maintains focus on attaining productivity standards and recommends innovative approaches for enhancing performance and productivity.
- Participates in the training and orientation of new colleagues as well as re-education for practice support.
- Serves as subject matter expert for access improvements, data tracking, and standard work.
- Works closely with ED clinicians and monitors patient lists to provide comprehensive care coordination for patients prior to leaving the Emergency Department.
- Works closely with hospital colleagues and monitors discharge reports to provide comprehensive care coordination for patients prior to being discharged from the inpatient units.
