Exciting opportunity to join a fast-paced medical team within a growing federally qualified community health center. HealthFirst Family Care Center, Inc. “The Heart of Our Community” provides comprehensive services including adult/pediatric medical care, health education, health assessment and screenings, gynecology, cardiology, behavioral health, substance use treatment, and adult/pediatric dental care. 


Certified Addiction RN Care Manager (CARN) Summary

The Addiction RN Care Manager will be responsible to engage patients accessing medication assisted treatment in the OBAT Program, provide skilled nursing care, integrated medical and behavioral health case management and coordination of treatment plans to patients who are being treated for opioid, alcohol, and other substance use disorders. In close collaboration with buprenorphine certified providers and behavioral health providers the Addiction RN Care Manager will provide and coordinate the daily and long-term care of patients in recovery. The Addiction RN Care Manager is under the general supervision of the Integrated Behavioral Health Program Director for clinical services.


Schedule:

Mon – Fri 8:30 am – 5:00 pm


Required Qualifications

  1. Registered Nurse (RN) degree from an accredited school or university and license to practice as an RN in the Commonwealth of Massachusetts. 
  2. 3-5 years of direct care/case management experience and a minimum of 1 year working in addiction/ substance use disorder treatment services. 
  3. An understanding of the relationship between substance use and mental health issues. 
  4. Knowledge of health, social service and funding sources.
  5. Appropriate continuing education.
  6. Must submit a CORI Release Form and demonstrate proof of being free from disqualifying information.
  7. Maintain current certification in CPR.
  8. Strong computer knowledge and proficient in Microsoft office applications and Electronic Medical Records (EMR).
  9. Bi-lingual skills preferred.
  10. Ability to handle confidential information in a professional manner and in accordance with HIPAA guidelines.

  4. Works well under pressure to manage stressful situations.
  5. Uses sound judgment and confidence.
  6. Ability to multi- task.
  7. Detail-oriented and accurate.
