About the role
DESCRIPTION OF POSITION:
- The HH Clinical Supervisor is responsible for analyzing data integrity and consistency of OASIS documentation and assessment processes. This position will ensure appropriate ICD-10 coding and sequencing will work with clinical staff to clarify documentation and data integrity issues. Assists Administrator with the supervision of care to patients in their place of residence.
SUPERVISION: Reports directly to the Home Health Director
RESPONSIBILITIES AND DUTIES:
- Reviews SOC/ROC/Recert/Transfers/Discharge assessments to ensure appropriateness, completeness, and compliance with federal and state regulations and organization policy.
- Ensures accuracy of POC development.
- Ensures appropriate ICD-10 coding and sequencing as it relates to the patient's medical condition.
- Consults with clinical staff to clarify any data integrity issues and works with clinician to make appropriate corrections per organization policy.
- Ensures Clinical record systems are maintained in compliance with state, federal, and ACHC regulations.
- Proficient with computers and Microsoft office.
- Knowledgeable of CMS and Home Health guidelines.
- Demonstrates ability to implement processes according to guidelines.
- Assists with QAPI, as required.
- "Owns" care of team of assigned patients.
- Participates in on-call as required.

EDUCATION AND WORK EXPERIENCE
- Graduate of an approved school of Nursing.
- Three to five years of nursing, one year of home health experience.
- Basic mathematical skills and ability to understand terminal digit filing system.
- Current Louisiana Registered Nurse license or multi-state compact license.
- Supervises care assigned team of patients and assists with supervision of field staff.
Physical Demands/Work Environment
- Work requires a variety of physical activities including standing and walking for long periods of time, bending, stooping, and lifting.
- The work environment may be physically confining.
- Visual acuity is required for a variety of specific activities.
- Must have normal hearing or utilize adaptive hearing devices if duties require listening with a stethoscope to discern breath sounds, heart sounds, and bowel sounds.
- Ability to do shift work as required.
