The program provides community-based therapy, case management and care coordination, and peer and family support through a team-based model. The whole team collaborates on one shared caseload, providing youth with an entire team of support, and allowing staff to share in the successes and challenges of their clients.</span></p> <p>The <strong>Intensive Care Coordinator (ICC) </strong>provides support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth’s natural supports together. This position<strong>&nbsp;</strong>is responsible for engaging and assisting foster youth and other clients, family members, foster parents, and other support persons in forming a Child and Family Team (CFT).
