About the role

A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patient’s condition that falls within their specialty area of practice. NOTE: Medical orders, progress notes and treatments plans are not required to be countersigned by a physician.

Nurse Practitioner- Stroke Center - Mount Sinai Brooklyn

Roles and Responsibilities

  • Develops and implements a collaborative practice agreement in the specific clinical area
  • Assesses the health status of patients by means of comprehensive health history and physical examinations
  • Initiates therapeutic and diagnostic procedures, consultations, and laboratory examinations.  Interprets diagnostic findings and accordingly alters plan of care including but not limited to prescribing and altering medication regimens and blood tests according to agreed-upon collaborative practice agreement and protocols in designated area.
  • Provides teaching and counseling in areas of health promotion and maintenance and in disease prevention in collaboration with Stroke Center Director and other Stroke Team members.
  • Clearly and comprehensively documents findings and plan of care in patient’s medical record, including but not limited to consultations, daily progress notes, and discharge summaries.
  • Collaborates with Stroke Center medical director, ED, and ICU providers regarding safe transfer of stroke patients to higher level of clinical care as deemed necessary.
  • Consults with attending Neurologists and Stroke Team members as established in the collaborative practice agreement.
  • Maintain knowledge current by self-directed learning and attendance at educational Programs and at least 8 hours of CME credits in stroke care management, prevention and related areas per year.
  • Participates in defining, maintaining, and interpreting standards of nursing practice.
  • Interfaces with Nurse Managers/Directors/ PA’s/ Nurses
  • Initiates hospital admission and discharge after collaborative consultation with Stroke Center physicians.
  • Obtains and maintains certification in NIH Stroke Scale
  • Responds to stroke codes in hospital, both in emergency department and any hospital unit requesting such services in a rapid and efficient manner.
  • Performs detailed history and physical examination on all consultations, obtaining data elements such as timing of studies and other parameters necessary for stroke center certification as specified by the stroke center director and / or stroke center coordinator.
  • Performs qualification of patients for acute stroke treatment, including intravenous thrombolysis with tenecteplase and endovascular thrombectomy.
  • Serves as a resource person to nurses and other team members during pre-hospital stroke notification and sustains support during acute active phase of stroke notification and recovery process to discharge. Serves as a resource for providing education to nursing staff of new evidence based practices supporting nursing practice goals.
  • Participates in departmental and interdisciplinary committees which influence and/or determine policies affecting nursing practice.
  • Participates in interdisciplinary clinical rounds and maintains log of rounding reports.

Requirements:

  • Will require Day/Evening Flexibility 
  • Graduate of an accredited Nurse Practitioner Program
  • National Certification as a Nurse Practitioner – ANCC or AANP
  • Current NYS licensure as Registered Professional Nurse, Current NYS Licensure as a Nurse Practitioner
  • Current Certifications in BLS, ACLS, PALS, Current DEA Certification or eligibility to apply
  • Current ED / Critical Care / Acute Care experience, Previous progressive experience relevant to area of clinical practice, Acute Care Physical & Neuro assessment skills, Expert in placement of peripheral IV placement

 

Compensation Statement

The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for this role is $87,692.00 - $131,538.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
