This job supervises the nursing staff who provide home visits to families with newborns, as well as the administrative staff who support the program. Ensures support and assessment of family needs in 12 domains of physical and psychosocial well-being and facilitation of referrals and connections to community agencies to support families in these areas. Supervises team of clinical and non-clinical staff to schedule, visit, and support families in the community using the evidence-based home visiting protocol.

Education

Required – Associate’s degree in nursing.

Preferred – Bachelor’s or master’s degree in nursing.

Work Experience

Required – 2 years of home care experience, including 1 year of experience in maternal child in a hospital setting.

Preferred – Clinical experience related to postpartum/child health, home visiting, lactation education, domestic violence, substance abuse and/or depression, maternal/child health, social work, or other related field.

Certifications

Required – Registered nurse (RN) license in state of practice.

BLS certification from American Heart Association.

Valid driver’s license in state of practice.

Preferred – Certified Lactation Educator or Consultant.

Proficiency in Spanish.

  • Leadership skills and ability to supervise a team with clinical and non-clinical staff.

  • Proficiency in using computers, software, and web-based applications.

Job Duties

  • Participates and supervises RNs in the education of families on postpartum/child health and lactation and conducting of home visits.

  • Participates and supervises RNs in providing a health and physical assessment of mother and baby in the home to determine well-being and assessment of risk factors for family, including depression, substance abuse, and domestic violence and appraising of situations and making decisions.

  • Participates and supervises RNs in the establishment of a trusting relationship with families during in-home visits and engagement of the family to establish a support plan with goals, objectives, and activities that address family strengths and needs.

  • Participates and supervises RNs in ensuring appropriate referrals and connections to community resources, identifying gaps in community services that may be addressed by local partners, and assisting parents in making and attending appointments for doctors, social service agencies, etc.

  • Acts as a liaison between families and other community agencies and serves as family advocate.

  • Participates and supervises RNs in responding to client needs through flexible scheduling of home visits.
