Position Summary: The Intake Associate plays a crucial support role in the home health intake process by assisting Intake Coordinators with administrative tasks. This position ensures smooth and efficient patient onboarding by handling data entry, scheduling, and follow-ups. The Intake Associate also conducts welcome calls after completing customer service and expectation training, helping to provide a seamless patient experience.
Essential Duties and Responsibilities:

Administrative Support:
- Assist Intake Coordinators with daily tasks to facilitate efficient patient intake and scheduling.
- Perform accurate and timely data entry in Salesforce, Electronic Medical Records (EMR) systems and logs.
- Maintain organized records of patient referrals and admissions.
Patient Engagement & Communication:
- Conduct welcome calls to newly referred patients.
- Provide a warm and informative introduction to home health services, addressing any initial questions.
- Communicate with referral sources, patients or family members to gather any missing or additional information needed for patient intake.
Scheduling & Coordination:
- Assist in plotting and scheduling initial patient visits.
- Communicate with clinical teams to ensure timely scheduling and visit confirmations.
Flexibility & Support:
- Provide coverage in various intake-related areas as needed.
- Adapt to workflow changes and support Intake Coordinators in meeting department goals.
Locations: We are hiring for the following locations: Burlingame, San Jose, Oakland, Fairfield, and Sacramento Offices.

Requirements
- One year of administrative experience in a medical setting is required.
- Strong organizational skills, attention to detail, proficiency in EHR systems, and understanding of HIPAA regulations.
- Excellent written and verbal communication skills.
- Proficiency in Google Suite and other relevant software.
