Your Responsibilities

• Conduct patient assessments and develop personalized Plans of Care (NO OASIS!)
  
  
• Monitor and document changes in patients’ conditions and needs
  
  
• Communicate proactively with patients and their families regarding care plans and health updates
  
  

Qualifications

• Current, unrestricted PT license in Texas
  
  
• Master’s degree in Physical Therapy (Required)
  
  
• CPR Certification & Negative TB Test (or Chest X-ray)
  
  
• Minimum one (1) year of experience in a home health agency, primary care clinic, or healthcare facility (preferred)
  
  
• Strong assessment, analytical, and critical-thinking skills
  
  
• Valid driver’s license & reliable transportation (must be willing to travel up to 30 miles)
  
  
• Ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull, as needed (must be able to lift up to 50 lbs)
