Summary

The primary function of this position is to provide cross-continuum care management services for high risk patients. Through collaboration with patients, providers and community resources, Care Navigators assess the needs of each patient and insure patients have timely access to the care they need.Current North Carolina Licensed Clinical Social Worker-Associate (LCSWA) required OR must be obtained within 60 days of employment. Must pass current North Carolina state licensing exam within 24 months of obtaining provisional license. Minimum 1-year Case Management and/ or geriatric experience preferred. Must have excellent written and verbal communication skills. Must be able to work independently, setting priorities to coordinate care plan efficiently as well as be able to work effectively in a team environment. Must be able to effectively communicate with physicians, patients, and their families and all levels of staff within the organization as well as with outside resources. Must have effective behavioral and educational strategies including, but not limited to motivational interviewing, teach-back method and self-management support. Must be able to recognize the role of cultural, social, economic and behavioral factors and respond to the diverse needs that result from these differences. Must have the ability to adapt quickly to changing patient needs and applicable organizational and federal regulations.Some light carrying and lifting may be required. Occasional walking may be required to access all areas of the Medical Center. Ability to effectively communicate orally to patients, family members, personnel and physicians. Near visual acuity to proofread hand or typewritten materials. Manual ability to use telephones and computer keyboard.
