About this role:   “Gatekeeper” is identified as an individual that is actively involved in the evaluation and planning of and treatment of committed individual beginning after the commitment through the planning of the individual’s discharge back into the community. This position is responsible for the oversight and implementation of referrals, monitoring while hospitalized, and discharge of adults and adolescents in need of state hospitalization. This position is responsible for the oversight and management of court commitments with Adult and Child.

The Location and hours: A&C is based in Central Indiana. For this position, we service the following county: Marion

Core Business Hours Monday-Friday

Responsibilities include but not limited to:

• Collaborates with referral sources within A&C as well as external community referrals
• Gathers necessary documentation and information including meeting with internal and external teams to gather clinical and historical information to support the application process. This may include community meetings with the referral source, assessment, consult with physician or psychiatrist, gather hospital records, and ensure court commitment is in place. 
• Coordinate with external providers, court entities, and supporting staff to ensure court commitments are managed as clinically appropriate. 
• Coordinate discharge into active treatment within A&C or an external treatment provider and provide assessments and supportive documentation post discharge to the ongoing provider. 
• Complete all documentation and communication within a timely manner. 

Requirements

 

•  Bachelors degree
•  Experience working with clients and families in clinic, school, home and community-based settings. 
•  Extensive knowledge of mental health field and hospitalization process. 
•  Extensive skills in child and adult observation and assessment. 
•  Experience and knowledge of systems for coordination of care.
