PATIENT SAFETY MANAGER

Job Summary
Under the direction of the Director of Patient Safety for Cook County Health (CCH) the Patient Safety Manager will use his/her background in patient safety, clinical knowledge and healthcare operations to review, abstract, and manage patient safety data in accordance with guidelines provided by CCH Patient Safety policies, the Patient Safety Organization (PSO) in which CCH is a member, regulatory bodies, and accepted patient safety experts. The Patient Safety Manager establishes and oversees patient safety data and trending reports for CCH; prepares and provides reports and analysis to organizational committees and departments; supports patient safety improvement activities; participates in patient safety educational activities; and provides patient safety training and education to CCH personnel.

General Administrative Responsibilities

Collective Bargaining
• Review applicable Collective Bargaining Agreements and consult with Labor Relations to generate management proposals
• Participate in collective bargaining negotiations, caucus discussions and working meetings

Discipline
• Document, recommend and effectuate discipline at all levels
• Work closely with labor relations and/or labor counsel to effectuate and enforce applicable Collective Bargaining Agreements
• Initiate, authorize and complete disciplinary action pursuant to CCH system rules, policies, procedures and provision of applicable collective bargaining agreements

Supervision
• Direct and effectuate CCH management policies and practices
• Access and proficiently navigate CCH records system to obtain and review information necessary to execute provisions of applicable collective bargaining agreements

Management
• Contribute to the management of CCH staff and CCH' systemic development and success
• Discuss and develop CCH system policies and procedures
• Consistently use independent judgment to identify operational staffing issues and needs and perform the following functions as necessary; hire, transfer, suspend, layoff, recall, promote, discharge, assign, direct or discipline employees pursuant to applicable Collective Bargaining Agreements
• Work with Labor Relations to discern past practice when necessary

Typical Duties
• Administers the patient safety program on a day-to-day basis by identifying safety concerns and works to correct them by developing safety plans with department managers.
• Coaches, mentors, and trains department manager on patient safety.
• Manages and analyzes patient safety data. Utilizes the EMERS system for reporting safety events or near misses.
• Facilitates performance improvement projects in patient safety to decrease and prevent safety events.
• Conducts patient safety educational programs, including new employees, residency orientation, monthly residency training, and error prevention training for all staff, etc.
• Cleanses data providing oversight, validation, and accuracy of all data submitted in the CCH safety event reporting system.
• Oversees safety processes with CCH operational and medical leadership.
• Complies with requirements of the PSO and regulatory bodies. Implements current standards and remains up to date on any changes.
• Reviews the Patient Safety and Evaluation System (PSES) protocols to support PSO protections.
• Formulates policy and organizational changes pertaining to patient safety improvements.
• Monitors patient safety program progress measuring primary metrics that pertain to the system as well as safety event specific measures.
• Works in cooperation with nursing, medical and other clinical or non-clinical staff to validate findings of patient safety investigation processes. Trains and/or teaches on how to recognize potential events to ensure a complete and accurate investigation is performed, i.e. cause analysis and human factor analysis.

Minimum Qualifications
• Bachelor's degree from an accredited college or university.
• Three (3) years of work experience in patient safety or clinical risk management within a healthcare environment.
• Two (2) years of experience working with Safety Event Reporting Systems and Electronic Medical Record Systems.
• Proficient with medical terminology. Preferred Qualifications
