* QUARTERLY INCENTIVE BONUS PROGRAM * 

 This position will cover correctional facilities and will require a supplemental background check conducted by the Ohio Department of Rehabilitation and Corrections.

Work Schedule: Monday – Friday 5am- 5pm Rotating Saturday at Toledo PSC 8am- 12pm. The hours/location may change/vary based on business need and/or the request(s) of your supervisor or management. 

Float Incentive: Additional $2.00/hr plus mileage reimbursement 

Work Location: Lima PSC Home Base 915 W market St, Lima, OH 45805 Traveling to North western Ohio areas. Possible for 2 hours travel time one way.

Job Responsibilities:  

• Perform blood collections by venipuncture and capillary techniques for all age groups 

• Collect specimens for drug screens, paternity tests, alcohol tests etc. 

• Perform data entry of patient information in an accurate and timely manner 

• Process billing information and collect payments when required 

• Prepare all collected specimens for testing and analysis 

• Maintain patient and specimen information logs 

• Provide superior customer service to all patients 

• Administrative and clerical duties as necessary 

• Travel to additional sites when needed 

Job Requirements:

• High school diploma or equivalent 

• Phlebotomy certification from an accredited agency is preferred 

• Previous experience as a phlebotomist 

• Must have a Valid Driver's License and good driving record 

• Must be at least 21 years’ old 

• Proven track record in providing exceptional customer service
