This is a hybrid position with 3-4 days onsite per week, meeting patients in clinic and supporting the careteam and 1-2 days remote per week following up with patients via phone and video.

 

 

Job Overview:

 

The Patient Resource Navigator provides navigation services to Blood Cancer patients with identified barriers. Navigation services include identifying and addressing barriers and connecting patients to available resources both in-house and within the community.  The Patient Resource Navigator focuses on identifying and mitigating concrete barriers to care (transportation, lodging, system barriers etc.) that may be limiting patients’ ability to participate in their treatment.  This Navigator role is crucial to support both Bone Marrow Transplant and Hem-onc patients with identifying and completing financial assistance grants, helping patients navigate lodging and transportation assistance programs, as well as supporting the transition of Hem treatment regimen patients from inpatient to outpatient and reducing hospital days.  This role works very closely with the Social Work team.

 

 

Responsibilities:

  • Assess patient barriers that may be impacting patients’ ability to participate in their treatment such as low health literacy, lack of understanding of diagnosis/treatment, psychosocial concerns, family/lack of support system, financial, housing/lodging concerns, transportation concerns, work concerns, language concerns, and nutrition/food insecurity concerns
  • Provide navigation services, guidance, and appropriate referrals face-to-face or via telephone
  • Perform a range of non-clinical care management functions (transportation and lodging coordination, outreach & research of available grant assistance programs, community resource assistance)
  • Navigate all patient referrals for concrete barriers through the healthcare system (priority areas include lodging and transportation). Utilize critical thinking skills to anticipate potential barriers and proactively work to address them
  • Organize, gather, prioritize, and integrate resources from various sources to provide comprehensive support
  • Interface with external organizations, as appropriate, to ensure cooperative efforts are enhanced and available resources are utilized; help patients navigate completion of financial assistance grants
  • Support clinical team with social determinant of health assessments and lodging and transportation support for priority Hem Treatment Regimen patients who we are transitioning from Inpatient to Outpatient care
  • Demonstrate an understanding of the larger, more complex issues of public health and their relationship to cancer care and patient outcomes

 

Patient Population:

All Blood Cancer Center patients (including Bone Marrow Transplant and Hem-Onc) needing assistance with concrete barriers, partnering closely with the Social Work and clinical team to identify and assess the patients with the greatest barriers to care.

 

Experience 

Two years of postsecondary education with three years of experience. 

OR AN EQUIVALENT COMBINATION OF RELEVANT EDUCATION AND/OR EXPERIENCE.  BS Social Work preferred. This is a patient facing role, strong organizational ability to manage multiple patients and different requests coming from the Social Work team. Epic experience. Hhealthcare and oncology experience a plus.

  • In-depth knowledge of department policies, procedures, and goals as well as administrative support in completing complex tasks.
