Job Summary:
The incoming Comprehensive Class Member Transition Program (CCMTP) Administrator will be responsible for managing incoming CCMTP referrals from Maximus. The Program Administrator will support program compliance with grant contract performance measures and timelines. They will collaborate with Trilogy intake and CCMTP Care Managers to ensure contact with Class Members in a timely manner. They will manage internal waitlists and triaging of referrals and available services. This position will be responsible for managing communication between internal staff to support collaborative and effective communication and workflows. This position will ensure compliance to data and reporting according to the IDHS grant requirements. They will manage the CCMTP hotline number and serve as the point of contact for external providers and Class Members assigned to the Trilogy Prime.
Essential Responsibilities:
- Ensure timely and seamless flow of Class Member referrals from Maximus to internal Trilogy Intake Team
- Coordinate with internal Intake Team to schedule intakes, communicate updates to departments, and make necessary adjustments to ensure scheduling and completion of intakes in a timely manner.
- Document and report on any referral issues to state partners and Supervisor as needed.
- Report on trends, community needs, and team updates to Supervisor.
- Coordinate caseloads and referrals to ensure the agency is meeting required timelines as outlined in our grant contracts.
- Return CCMTP hotline calls. Educate class members and callers about the program, communicate expectations regarding the intake process, eligibility for services, and organization services provided.
- Communicate and collaborate effectively with program leaders, state or other external partners, etc. and problem solve barriers and bottlenecks to support efficient linkage to care.
- Track updates and progress in internal systems, as well as state monitored systems such as Caspio and AssessmentPro.
- Provide direct supervision and oversight to program assistants and administrative assistants.
- Facilitate direct and consistent communication between administrative teams and clinical teams to support collaboration and efficient workflows.
- Identify opportunities for consolidation, cross training, and shared responsibilities to ensure all programs have consistent high-level coverage for administrative tasks.
- Ensure coverage of critical tasks across all Admin Assistants.
- Facilitate regular CCMTP admin meetings to ensure consistency and streamlined processes across teams.
- Coordinate and manage Class Member transfers to/from Prime agencies to ensure all information is collected and distributed to the appropriate parties.
- Facilitate monthly contact with assigned SMHRF/SNF facilities.
- Represent Trilogy in a positive and professional manner; and communicate effectively and professionally to others about potential organizational barriers/issues.
- Perform other related duties and/or projects and participate in committees/workgroups as assigned.
Qualifications
- Demonstrated work experience in an administrative, clerical, or site management position
- Bachelor’s Degree Preferred
- Demonstrated ability to train others in administrative processes
- Thorough knowledge of Microsoft applications (Outlook, Teams, Word, Excel, etc.), report generation, and database management tools
- Excellent customer service and organization skills
- Prior experience in mental health field preferred
- Prior leadership experience preferred
