As a member of the Community Health team, this leader works to improve the health of the community, with a focus on low-income and underserved communities and collaborative efforts with public and private partners.

Position Details:

The Senior Community/Population Health Consultant provides consultation to leadership in the areas of healthcare delivery transformation, healthcare reimbursement, community health, and internal and external collaborative opportunities. Incumbent will manage large and complex projects in healthcare transformation efforts with minimal direction.

This is a full-time, day shift role with some evenings, weekends, and travel required. This is a remote role with some required in-person activities. The successful candidate can live in any of our primary service areas which include Montana, Idaho, Utah, Colorado, and Nevada.

Job Essentials:

  • Manages large and complex initiatives, acting as a subject matter expert, in healthcare delivery transformation, healthcare reimbursement community health, and internal and external collaborative opportunities.
  • Identifies, analyzes and monitors industry, regulatory, technology and market-based trends that impact Population and Community Health.
  • Performs analysis and recommendations on new programs and/or changes to existing programs.
  • Constantly feeds new ideas in innovation and strategy (including community benefit reporting, SDoH, payment innovation, digital solutions, collaboration, commercial products, wellness, etc.).
  • Organizes, optimizes, and communicates progress toward the completion of the overall work portfolio utilizing a project consultant as a resource.
  • Acts as a thought leader and consultant to leadership in Population Health and Community Health.
  • Communicates and shares the vision for Population Health and Community Health to other key internal and external stakeholders as requested.


Minimum Qualifications:

  • Bachelors degree. Education must be obtained through an accredited institution. Degree will be verified.
  • Seven years of healthcare experience, with three in management, with track record of success.
  • Strategic thinker with the ability to articulate a clear vision and develop strategies and implementation plans that include building support among a diverse set of constituents that may have competing objectives.
  • Demonstrated effectiveness in a matrix organization.


Preferred Qualifications:
Experience and expertise working with public health data to identify health needs.
Prior success in developing and implementing effective health improvement strategies.
Ability to lead through others and advise/work with diverse groups of caregivers and leaders, including leaders from outside Community Health.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$57.75 - $155.92
