Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary

The Behavioral Life Coach is responsible for providing comprehensive social work services to individuals and their families to find practical ways of overcoming social, emotional, and environmental barriers. They will provide education for Ventanilla de Salud (VDS) participants and individuals, families, and community regarding mental health awareness.  Conduct mental health assessments and submit referrals to UNAM- mgGAP and local low-cost mental health providers.

Essential Functions and Responsibilities

•    Proactively contributes to Eskenazi Health's mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County. Models Eskenazi Health values
•    Documents incoming calls and follows up with individuals
•    Conducts weekly workshops, educational presentation on Mental Health stigma, and promotes health awareness in front of a live audience at the Mexican Consulate in Indianapolis
•    Records educational mini workshops on video to be shared with individuals 
•    Posts recordings on social media to help individuals understand mental health 
•    Conducts behavioral assessments in a culturally and sensitive manner to identify individual’s behavioral needs
•    Submits referrals to mental health providers and UNAM-mhGAP
•    Basic needs referrals to community resources 
•    Participates in radio or tv interviews as needed for mental health awareness, and ways to cope with everyday stressors  
•    Assists with development of new programs and initiatives
•    Utilizes interpersonal communication strategies/skills to achieve desired outcome with participants/families and others
•    Responds to others in a positive, constructive, timely and courteous manner
 

Job Requirements

•    Certification as Behavioral Life Coach
•    Two+ years of experience managing and integrating social/community needs 
•    Experience in a managed care environment 
•    Valid driver’s license
•    Bilingual preferred

•    Effective communication skills
•    Ability to work cooperatively with multi-discipline teams in situations requiring a high degree of flexibility, creativity, initiative
•    Ability to perform duties independently under the direction of a specified supervisor
