About Us
Avivo’s Ending Homelessness division is dedicated to creating pathways out of homelessness through a Housing First and harm reduction approach. We partner with county systems, tribal governments, and operate innovative street outreach and housing programs—including Avivo Village, a culturally responsive, low-barrier shelter designed to help people transition to permanent housing. Our programs support individuals and families in both scattered-site and site-based housing using trauma-informed, person-centered practices that promote dignity and stability.
Your Role
As a Case Manager within our scattered site housing programs, you’ll work closely with individuals and families experiencing homelessness to stabilize and transition into housing. You’ll meet clients where they are—literally and figuratively—whether that’s in shelter, community, or temporary housing. You’ll support them in developing housing goals, connecting to resources, building life skills, and ultimately securing and maintaining housing. This position plays a vital role in supporting participants referred from Hennepin County family shelters. This position is 32hr/week.
Key Responsibilities Include
- Provide monthly face-to-face case management and creative outreach with assigned caseload
- Build trusting, trauma-informed relationships to support client engagement
- Develop housing plans tailored to individual and family needs and strengths
- Assist clients in obtaining vital documents and connecting to medical, mental health, and recovery services
- Support clients with housing readiness, tenant rights education, and eviction prevention
- Mediate landlord relationships and assist with navigating housing crises or transitions
- Accurately document services and complete all required case notes, assessments, and reports
- Collaborate with team members, supervisors, and external providers to coordinate care
- Actively participate in undoing racism efforts and ensure services are culturally responsive
Qualifications
What You Offer
- High School Diploma or GED required; Associate degree or higher preferred
- One year of experience working with individuals experiencing homelessness—or relevant lived experience
- Strong communication skills and ability to work with people from diverse backgrounds
- Knowledge of Housing First, harm reduction, trauma-informed care, and person-centered practices
- Able to build rapport, engage in crisis intervention, and navigate community resources
- Organized with strong documentation skills and attention to compliance
- Must have access to reliable transportation, valid driver’s license, and car insurance
