The incumbents may be requested to perform job-related responsibilities and tasks other than those stated in this job description. </span></p> <p><span> </span></p> <ul> <li><span>Interview and ascertain the needs of applicants seeking housing assistance and determine program eligibility.</span></li> <li><span>Provide ongoing case management to clients by developing and monitoring individualized case plans designed to help clients achieve the highest level of self-sufficiency as they move through the Continuum of Care.</span></li> </ul> <ul> <li><span>Provides follow-up case management services for a period of at least one year.</span></li> </ul> <ul> <li><span>Identify and utilize additional community resources that will help clients. Refer clients to these services.</span></li> </ul> <ul> <li><span>Record all data in </span><span>electronic client management database.</span></li> </ul> <ul> <li><span>Maintain financial guidelines when providing assistance in order to stay within budgeted allotments.</span></li> <li><span>Reports monthly statistics and annual outcomes to the Director of Social Services.</span></li> <li><span>Build and maintain rapport with community agency representatives and housing providers to ensure collaboration within the community.
