The incumbents may be requested to perform job-related responsibilities and tasks other than those stated in this job description. </span></p> <p><span> </span></p> <ul> <li><span>Interview and ascertain the needs of applicants seeking housing assistance and determine program eligibility.</span></li> <li><span>Provide ongoing case management to clients by developing and monitoring individualized case plans designed to help clients achieve the highest level of self-sufficiency as they move through the Continuum of Care.</span></li> </ul> <ul> <li><span>Provides follow-up case management services for a period of at least one year.</span></li> </ul> <ul> <li><span>Identify and utilize additional community resources that will help clients. Refer clients to these services.</span></li> </ul> <ul> <li><span>Record all data in </span><span>electronic client management database.</span></li> </ul> <ul> <li><span>Maintain financial guidelines when providing assistance in order to stay within budgeted allotments.</span></li> <li><span>Reports monthly statistics and annual outcomes to the Director of Social Services.</span></li> <li><span>Build and maintain rapport with community agency representatives and housing providers to ensure collaboration within the community. Attend monthly Brown County Homeless and Housing Coalition meeting, Brown County Social Services Providers meeting, Coalition subcommittee meeting, and other meetings as assigned.</span></li> <li><span>Participates in professional development opportunities related to position.</span></li> <li><span>Participates in seasonal activities/special events as requested by supervisor or Salvation Army Officer(s).</span></li> </ul> <p><strong><span> </span></strong></p> <p><strong><span>Minimum Qualifications: </span></strong></p> <p><strong><span> </span></strong></p> <p><span>The requirements listed below are representative of the minimal education, experience,  skills, and/or abilities required for this position.  </span></p> <p><strong><span> </span></strong></p> <ul> <li><u><span>Education</span></u><span>: Bachelor’s Degree in Social Work or related field.</span></li> </ul> <p><span> </span></p> <p><u><span>Experience:</span></u><span>   Two years case management experience with experience in mental health and/or AODA experience preferred</span></p> <p><span> </span></p> <p><span>-OR-</span></p> <p> </p> <p><span>Any equivalent combination of training and experience which provides the required knowledge, skills and abilities.</span></p> <p><span>   </span></p> <p><u><span>Certi
