Substance Abuse Counselor - Outpatient

Burkwood Treatment Center is seeking a WI licensed Outpatient Counselor to provide treatment and support to assist patients in their recovery from addictions, such as alcoholism and drug addiction. 

ESSENTIAL JOB FUNCTIONS:

• Conduct individual and group counseling sessions in an IOP (Intensive Outpatient) setting. 
• Responsible for initial assessments, as well as follow-up assessments for patients.
• Prepare individual treatment plan for each patient.
• Maintain patient evaluations, notes, and reports as required.
• Provide case management duties for patient, ensuring individualized quality care.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

• High school diploma or equivalent required.
• Degree in social or health services field from an accredited college or university required.
• License in the State of Wisconsin as and SAC or CSAS required.
• Flexibly and has the ability to assist at Residential Treatment.
