Team Registered Nurse | Home Health

Find your purpose with CentraCare by treating patients where they’re most comfortable – in their home. As a Home Health Team Registered Nurse, you will work independently providing skilled care to patients across our CentraCare territory.  Common duties include wound management, infusion therapy, assessments, education and training for patients on their new diagnosis, and more. We provide extensive training and ongoing RN office support while you’re making home visits to patients. 

Schedule

• Casual | At minimum, every 4th weekend & two holidays per year
• Part-time hours required through orientation
• Day shift | 8:00 am – 4:30 pm 

Pay and Benefits

• Pay based on experience and degree
  • Associate/Diploma in nursing starting at $37.27 per hour
  • Bachelor’s in nursing starting at $38.20 per hour
  • Master’s in nursing starting at $39.16 per hour
  • Pay range: $37.27 - $55.91 per hour
• Milage reimbursement and extra pay for working weekend shifts

Qualifications

• Graduate from an accredited school of nursing 
• Current Minnesota Registered Nurse license 
• Current American Heart Association (AHA) BLS Healthcare Provider card required
• Reliable transportation, valid Minnesota driver’s license, a driving record in good standing, and insurance liability coverage required
• Minimum of one year of RN experience or 2 years experience in a Home Health setting  
• Experience in Medicare Certified Home Health Agency preferred 

Based out of our office in Marketplace Mall, the Home Health department primarily takes place in the communities we serve, in the homes of our patients. Our staff provides services not just for St. Cloud but our surrounding areas as well including Buffalo, Alexandria, Little Falls, and more. Teams are assigned to areas to minimize driving.
