About the role

The Licensed Vocational Nurse (LVN) is responsible for delivering safe, effective, and individualized nursing care to pediatric patients with complex medical needs in a private duty nursing (PDN) setting. The LVN executes the nursing process including assessment, planning, implementation, and evaluation under the supervision of a licensed Registered Nurse or Clinical Supervisor. This role ensures continuity of care and supports the interdisciplinary team in achieving exceptional outcomes for patients and families.

Essential Duties & Responsibilities

  • Provide skilled nursing care in accordance with the patients individualized plan of care and physician orders.

  • Perform ongoing clinical assessments and report changes in patient condition promptly to appropriate supervisory personnel.

  • Support and reinforce the plan of care by collaborating with direct care staff, ensuring accurate implementation of prescribed treatment.

  • Educate patients and families on aspects of care, diagnosis, medication administration, and preventative health strategies.

  • Communicate and collaborate with physicians, therapists, and other healthcare professionals to deliver high-quality, coordinated care.

  • Attend and participate in all required meetings, including team conferences, staff meetings, and continuing education in-services.

  • Document care provided accurately, timely, and in accordance with agency policy, state and federal regulations, and national accreditation standards.

  • Comply with clinical supervision protocols and participate in performance improvement initiatives.

  • Provide care in patient homes and/or center-based settings as directed.

  • Perform additional nursing duties as assigned.

Qualifications & Requirements

  • Licensure: Current and unrestricted Licensed Vocational Nurse license in the State of Texas.

  • Certification: Current CPR/BLS certification (must be maintained throughout employment).

  • Experience:

    • Minimum of one (1) year of clinical nursing experience preferred.

    • Pediatric and/or home health experience strongly preferred.

  • Transportation: Must possess a valid drivers license, proof of current auto insurance, and reliable transportation to travel between the office and patient homes.

Education Requirements

  • Graduate of an accredited vocational nursing program (Diploma, Associate, or Baccalaureate).

Physical Demands

  • Ability to lift and carry up to 50 pounds.

  • Physical stamina to perform duties including prolonged standing, bending, patient handling, and operating clinical equipment.
