JF&CS is now seeking a Part Time License Practical Nurse (LPN) for 20 hours per week.
PURPOSE:To provide oversight of health care management for clients in the Developmental Disabilities Services Division.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Provide health care oversight and management, and documentation to maintain compliance with agency policies & procedures, Developmental Disabilities Medicaid waiver services, CARF standards and all regulations for clients in the Zimmerman Horowitz Independent Living Program and Day Support program.
  • Conduct and document supervisory home visits at least every 60 days, and as needed.
  • Conduct and document annual skill checks for all staff as needed.
  • Provide health care oversight to Day Supports clients as needed.
  • Coordinate medication management for individuals in the Zimmerman Horowitz Independent Living Program.
  • Consult with staff, clients, family, medical professionals to facilitate health care management for clients.
  • Attend Individual Service Plan meetings for clients as needed.
  • Conduct in-service trainings as needed for staff.
  • Monitor annual physicals and coordinate appointments.
  • Maintain client care plans, physicals and supervisory home visit schedule.
  • Ensure health care protocols are up to date for all clients.
  • Provide follow up supervision with clients and staff to resolve client-related issues.
  • Promptly Report any health-related issues discovered during 60-day assessment or relayed by staff to RN.
  • Manage On-call Nursing Emergency Cell Phone in rotation with Dept RN, including overnight, weekend and vacation coverage.

Requirements

  • Maintain active and in good standing current Georgia LPN License.
  • Strong interpersonal and communication skills.
  • Ability to be a good team player.
  • Excellent organizational skills.
  • Ability to work independently.
  • Maintain active and in good standing current Georgia driver license.
  • Maintain 16 hours of DBHDD annual training.

PHYSICAL DEMANDS:

High energy level; ability to get along with diverse population; tactful; mature; flexible. Ability to manage multiple tasks and assignments; ability to concentrate and focus on information, reports and outcome measures; and comfort working in a high performance team environment. Physical requirements include sight, hearing, sitting for more than four hours each day, lifting 50 lb or less (per HRST requirement) and other physical requirements required to complete essential functions of the position. Must be able to work a flexible schedule.


WORK ENVIRONMENT:

May vary from site to site as the employee travels frequently making home visits, meetings and events.


EDUCATION and/or EXPERIENCE:

  • 5-year experience as a Licensed Practical Nurse.
  • Experience with developmental disabilities preferred.
  • Current Georgia licensure as a licensed practical nurse.
  • Computer proficiency.
