Your Specific Responsibilities:

This position is responsible for detecting, controlling and preventing Sexually Transmitted Diseases including HIV and Hepatitis for Polk, Highlands and Hardee Counties.  This incumbent is assigned to the clinic/field team and may be required to work a flex schedule evening and weekends as program needs dictate.  This position is classified as sensitive and governed by state and local confidentiality and security policies in accordance with Florida Statues 384, Florida Administration 10-3 and Health department guidelines, rules and regulations.  Incumbent is responsible for maintaining and securing confidential data sets: e.g. STD/HIV case reporting, surveillance information and clinical information.  Incumbent is responsible for following all Internal Operating Policies, Procedures, and other specific supplemental operating procedures related to the position.  Incumbent must know venipuncture procedures and incorporate into daily field activities.  Incumbent is expected to communicate in a courteous and professional manner always all the time.   Incumbent Is required to review client information entered into HMS Clinical Encounters, including progress notes/labs and scheduled appointments, and enter accurate EAR and other work documentation in a timely manner.

 

  • Assigns cases from the ELR to Field Staff every morning.
  • Pulls faxes from confidential fax, uploads documents to STARS, updates record including adding labs, conducts preliminary investigation on case, including calling provider for treatment information, then assigns case to DIS for further follow up.
  • Consults with private physicians, private and public laboratory personnel, and other health care providers to stimulate case reporting, obtain information on STD patients/contacts/clusters, and share information on diagnosis and treatment as appropriate.  Provides screenings, educational presentations and in-service training as directed/needed. Collaborates with community partners and performs outreach activities.
  • Coordinates and conducts timely personal interviews and re-interviews of patients infected with sexually transmitted disease (STDs) and HIV/AIDS for purpose of educating patients regarding their infection and appropriate medical care, developing plans to locate and refer their sex and needle sharing partners and others at risk for infection to a medical facility for examination and appropriate treatment.  Performs cluster interviews on persons determined to be potential information sources of known cases.  Utilizes HMS, STARS, and other available computer applications for finding locating information for stalled patient investigations.
  • Provides confidential counseling, testing and referral services for named contacts of STD and HIV infected persons upon referral from private physicians, hospitals, local clinic and other sources.
  • Will assess STD clients for PrEP and refer candidates for PrEP to closest health department clinic or other appropriate health care organization. Will provide information regarding the use of PrEP to patients who may benefit from PrEP.
