Hospice RN Case Manager

Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Service Area: Saint Johns to Mt Pleasant (North Territory)

Your Impact

As a Hospice RN Case Manager, you will ensure comfort and compassion guide every interaction. Your responsibilities include:

  • Holistic Patient Care: Perform physical, emotional, and environmental assessments to create individualized care plans.

  • Meaningful Visits: Deliver hands-on nursing care, ensuring every interaction supports the patient’s well-being.

  • Team Collaboration: Communicate vital updates to supervisors, physicians, and care team members to align care strategies.

  • Empowering Families: Educate and support loved ones, offering guidance and resources to navigate their journey.

  • Proactive Response: Monitor and address changes in condition promptly, ensuring the patient’s care needs are always met.

  • Commitment to Excellence: Follow all care plans and physician directives, delivering care that prioritizes dignity and quality.

What You’ll Need

  • Graduation from an accredited nursing program (Certificate, Diploma, Associate, or Bachelor’s).

  • Current RN license in Michigan.

  • A minimum of 1 year of clinical nursing experience (hospice or home health preferred).

  • Reliable transportation for patient visits.

  • A dedication to multitasking, documentation, and meeting deadlines while delivering outstanding care.
