As a Regional Director of Clinical Services (RDCS) at Transitions Care you will be accountable for developing, training, coaching, and mentoring the Director of Nursing across a designated region to build and sustain a team of strong, high-performing Director of Nursing’s. This role maintains direct accountability for established metrics and KPIs — including employee turnover, CAHPS scores, live discharge percentage, and visit utilization in addition to other metrics as appropriate — and holds the Director of Nursing accountable to achieving these outcomes at every location within the region.


This position will cover Michigan and Ohio. You will need to be licensed or able to obtain your RN license in both states.


Major Responsibilities

Director of Nursing Accountability & Development

  • Trains, coaches, and mentors Director of Nursing to develop strong, accountable clinical leadership capabilities at every location within the region. Leadership development is an ongoing, structured process — not a one-time event.
  • Sets clear, measurable clinical goals and performance expectations for the Director of Nursing and Administrator at all locations overseen, establishing the regional standard for clinical excellence.
  • Holds the Director of Nursing tightly accountable to established metrics and KPIs, including: employee turnover, CAHPS patient and family satisfaction scores, live discharge percentage, and visit utilization or other key metrics as identified through the QA process. The RDCS monitors these metrics consistently and drives leadership action toward sustained improvement.
  • Strategizes and develops individualized improvement plans for locations that are not meeting clinical performance targets, supporting the Director of Nursing in executing those plans with urgency and structure.
  • Does not directly manage the Director of Nursing (that accountability rests with the Administrator), but formally reports performance concerns, gaps, and lack of progress to the Administrator, escalating to the Area Vice President as appropriate.
  • Partners with the Area Vice President on specific strategic goals and objectives, including survey plans of correction, while independently owning clinical outcomes across the region.
  • Backfills for the Director of Nursing during vacancies to ensure teams are not without clinical leadership, maintaining operational and quality continuity.

Clinical Quality Feedback Loop

  • Owns and manages the clinical quality feedback loop across the region: the RDCS identifies clinical performance gaps in the field, the QA team validates those gaps, and the Education team delivers targeted interventions to resolve them. The RDCS is accountable for ensuring this loop operates as a continuous, closed cycle — not a series of isolated activities.
  • Functions as the primary change agent for clinical gaps identified through the QA process, ensuring that identified deficiencies result in concrete corrective action and that improvement is verified and sustained.
  • Works collaboratively and continuously with the Quality Assurance team to ensure accurate gap identification, timely follow-up, and clear communication between field staff and the QA and Education teams.
  • Evaluates the effectiveness of educational interventions against clinical outcome data, ensuring that education translates into measurable improvement and does not simply satisfy a compliance checkbox.
  • Ensures all sites maintain a state of survey readiness at all times, proactively identifying vulnerabilities and driving corrective action before deficiencies are cited.

Clinical Operations & Compliance

  • Ensures Director of Nursing’s have a strong working understanding of the admissions and recertification process, including eligibility criteria, and holds them accountable to applying that knowledge consistently.
  • Assists in the development and ongoing evaluation of effective staffing structures to achieve optimal coverage and sustain quality standards across all locations.
  • Provides guidance and oversight on budget management items including staffing, medications, nursing supplies
