This is a part-time position working 20 hours a week Mon-Fri between the hours of 8am-5pm. Both 4 and 8-hour shifts available.
MUST have a masters degree in social work and have a current Missouri license of LMSW or LCSW.
Job Summary:
Provides case management and care coordination to pregnant women with substance use disorder throughout thier pregnancy and up to two years after the birth of the child.Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
- Connects the mother with reliable referrals in the community to support meeting the basic biopsychosocial needs of mother and infant.
- Connects mothers to health systems and community organizations for well-baby checkups, meeting development milestones, as well assessing for a safe and healthy home environment.
- Builds a trusting relationship with the mother and provides coordination between inter-disciplinary team, health systems and community organizations in meeting the medical needs of mothers and their children are also receiving their well-baby checkups, meeting development milestones, as well as have a healthy home environment for the infant to thrive.
- Works closely with Children's Division to coordinate disposition of baby upon discharge from hospital.
- Develops individual plan of care in coordination with the inter-disciplinary team that focuses on client needs through evidence-based interventions.
- Maintains documentation of activity, programs and participation.
- Collects and evaluates data to reach and maintain goals of program.
- Works in a constant state of alertness and safe manner.
- Performs other duties as assigned.
EDUCATION
- Master's degree in Social Work
EXPERIENCE
- One year experience
PHYSICAL REQUIREMENTS
- Frequent reaching, sitting, standing and walking.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more).
- Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
- Frequent keyboard use/data entry.
- Occasional bending, stooping, climbing, twisting and gripping.
- Occasional lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Occasional use of vision to identify and distinguish colors.
- Occasional driving.
- Rare repetitive foot/leg and hand/arm movements and squatting.
