Where you’ll see your impact: As a Remote Pulmonary Rehab Nurse, you will specialize in supporting patients recovering from heart and lung conditions. Your impact will be deeply felt through meaningful patient relationships, individualized care planning, and education that helps prevent readmissions and empowers long-term wellness. You’ll play a crucial role in both pulmonary and cardiac rehabilitation, helping patients improve their cardiopulmonary health and quality of life.
What you’ll be responsible for:Pulmonary Rehab Classes:Lead virtual group pulmonary rehab classes (3 classes per day, Tuesdays & Thursdays at 9, 11, 12:30 with 8 pts in each class).Use established curriculum and structure to avoid disruption to current class schedules.Create a supportive, engaging environment for patients participating in live sessions.
Individualized Intake & Care PlanningConduct 1:1 intake assessments for new patients.Develop personalized ITP Checklist from AACVPR and care plans.Evaluate readiness and fit for class-based rehab programs.
Bonus: Cardiovascular Education & SupportProvide patient education on heart disease management, including: Diet and nutrition Exercise and lifestyle changes Medication adherenceOffer support to patients and their families as they adapt to life with cardiovascular conditions.
Hospital Discharge Coordination // TriageEnsure seamless transitions for patients being discharged from the hospital 3-5 patients/day- with follow up with them for 3 visits (3 weeks).Coordinate necessary equipment, medication, and follow-up plans.Collaborate with care team members to ensure continuity of care.Provide triage coverage.Manage symptom escalation and reduce dependency on after-hours services.Support timely intervention and resolution of patient concerns utilizing standing orders.
You would be a good fit:Direct experience building and/or leading a Pulmonary Rehab ProgramAn active and unrestricted Registered Nurse (RN) license- COMPACT/MULTI STATE in addition to New York and CT (bonus)Bachelor of Science in Nursing (BSN) preferredStrong background in pulmonary care, rehab, or a related fieldComfort and confidence working in a fully remote settingExcellent communication and interpersonal skillsA compassionate, patient-centered approach to careFlexibility and team-oriented mindset – you’ll be working closely with internal care teams across multiple departments
